Use this form if you have purchased licenses for Exinda Virtual Appliances. This is not for trial licenses.

To use this form you must have

• Your Purchase Order number
• The Exinda Virtual Appliance's Host ID(s)
• The hypervisor type where you have installed the Exinda Virtual Appliance
• The License level for each of the virtual appliances - you can get this from your purchase order

NOTE: To acquire the Host ID information you must:

1. Install the Exinda Virtual Appliance;
2. Open a browser to the IP address of the newly installed virtual appliance and find the Host ID on the Dashboard tab.

Once you have captured this information you can proceed with completing this form. This information will be sent to the Exinda Accounts team to process and issue your software license certificate(s).

• For each additional Host ID enter the "Add New Row" button to create a new line.
